This screening application is for people interested in adopting and animal from GoodFellas Rescue and stays on file for 6 months. It is to be used as a pre adoption application that contains valuable information on the adoption process, what we look for at our home check and what we expect of our adoptive parents. Thank you for your interest in a GoodFella Animal.

Be advised that though we prefer a fenced yard, we understand that every situation is different and we are willing to take that into consideration. It is judged on a case by case basis and will be determined after we do a home check. Factors we consider is the size of the dog, the activity level of the adopters, location of home and responsibility level of adopters along with previous experience and references. Also be advised that all animals are chipped with the rescue information and if the animal turns up at a shelter or pound, the dog will be confiscated by GoodFellas Rescue and will not be returned to the adopter unless the situation and circumstances were in no way related to lack of fencing in the yard.

What we look for at our Home Checks:

• Location of home including the traffic on the near by roads.
  

• General Safety of the home and the yard.
  

• The health of any animals already in the home.
  

• The general layout as well as how suited the areas the animal will be kept in are.
  

Here is a preview of the things we ask our pet parents to adhere to for the final agreement:

• I, the adopter, expressly agree to never sell, lease, or give away the above described animal for the duration of the animal's life without first notifying G.F.R. If at any time my life situation has changed and I must re-home the animal, I, the adopter, will notify G.F.R before taking any action to re-home the animal.
  

• I, the adopter, agree to provide adequate shelter, good quality food, clean water and attention to the above described animal for the duration of the animal's life.
  

• If I, the adopter, can not longer provide these things to the animal I agree to bring the animal back to G.F.R for care and re-adoption.
  

• I, the adopter, agree that at any time during the animal's life, a representative of G.F.R can ask to visit the animal and that if the animal is not receiving the proper care and its situation is considered neglectful abuse, can be removed from the property and placed back into G.F.R's care.
  

• I, the adopter, agree to never use the above described animal for illegal purposes such as dog fighting, baiting or any other violent sport that results in or may result in injury or loss of life to the above described animal. If such an event is suspected, I, the adopter, expressly understand that I may be brought up on criminal charges as well as have the animal removed from the residence.
  

• I, the adopter, accept full legal responsibility for any damage to property, animal or person that may be caused by the above described animal after signing this final adoption contract and will in no way hold G.F.R responsible for those costs.
  

• I, the adopter, understand that G.F.R has disclosed any and all information known to them about the temperament and disposition of the above described animal and has not withheld said information at any time during the adoption process.
  

• I, the adopter, understand that the above described animal has been given vet treatments and vaccinations up to the date of adoption and if there is more work or vaccinations needed in the series, it is up to the adopter to finish it promptly.
  

• I, The adopter, understand that the adoption fee of $150 is considered a Minimum donation to G.F.R and is in no way refundable regardless of the situation or circumstances.

Where will the animal be housed? Please keep in mind that we do not adopt Outside Dogs, We require all animals adopted to be family dogs that are housed primarily inside with the family.
